National Health Insurance

National Health Insurance is a proposed universal health coverage system in South Africa designed to provide all citizens and legal residents with access to essential healthcare services, regardless of their employment status or ability to pay. The scheme aims to pool public healthcare funding and purchasing power to achieve equity in healthcare access and reduce the financial burden of medical costs on individuals. Under the model, the National Health Insurance Fund would purchase healthcare services from both public and private providers on behalf of the population, with the goal of eliminating out-of-pocket payments at the point of care. The system is intended to be funded primarily through general taxation, with contributions from individuals and employers phased out over time. Implementation has been structured in phases, beginning with strengthening public health facilities and systems, followed by the establishment of the fund and contracting mechanisms. The initiative represents a fundamental restructuring of South Africa's two-tier healthcare system, which has historically seen well-resourced private care serving a minority alongside under-resourced public services for the majority. Key policy debates have centred on fiscal sustainability, the role of medical schemes, provider readiness, governance structures and the constitutional implications of centralising healthcare purchasing. The National Health Insurance Bill has progressed through Parliament amid significant opposition from provincial governments, medical schemes, healthcare professionals and civil society organisations. Proponents argue the scheme is essential to realising the constitutional right to healthcare and addressing deep inequalities, whilst critics raise concerns about affordability, implementation capacity and potential impacts on healthcare quality. The policy has remained a priority for national government despite fiscal constraints and ongoing legal challenges.
